How can i chose the categories of related item(one item per category) and exclude current category.

When displaying item1 from category1 related items by tags to be display: item2 from category2, item3 from category3, item4 from category4
Do not display any item from category1 or two items from category 2, 3 or 4

Any help how to change item.php to do this?

You will need a custom plugin for this which gives you more control over the related items.

You can browse the JED or getk2.org/extend/extensions
