both.. Is this possible in k2? how do I do it? I want to choose which users see k2 items that I create under any clients category

Oh I misread the question.
Yes you can use Joomla!'s ACL in K2 categories and items (menus as well) to manage who gets to see your content.

I know you can do that in joomla, but thats now what I want..

I want each registered user to be shown 1 item that will be assigned to them. for example

registered user 1 sees ..... Item 1
registered user 2 sees ...... Item 2
etc.

This is not possible by default. Items can be shown to user groups not specific users.

how do I do I display item 1 to user group 1? there is no options to select user groups when adding a new item.

Check the Access level field in the item's form.
Its located bellow the author related fields.
