Although I have setup all things correctly like enable "prepare content"in plugin parameters, use the right tag, etc... my MP4 video isn't loading at all.

See settings:

tag module custom: {mp4}bowlhouse{/mp4}
plugin parameter video path: images/stories/videos

Module positioned on homepage.

Any idea what is causing such thing? Did I left something behind?  8)

Make sure that your mp4 file is encoded in h.264
