Hello Jordi,

Make sure that you have selected the Plain option as your contact's Display format through the general contact parameters in the Contact tab. Then go to the contact you have created and add the info of your preference.

The map is published in "numuzik_above_component" module position in a K2 Tools -> custom code module assigned to this page.

Your contact form seems fine. When you put the info where does it show? Have you put the address, phone, etc to see where the block is shown?

The info block is included in a block which is controlled by the contact position and vCard.
Enable the contact position option for now and we will include a fix for this in an upcoming update of the template.
