Is there a way to for users to report a dead link in K2 (link extra field).

I'm running an anime ddl site (www.animekyun.com) and i link to megaupload and stuff. But obviously those links break usually and I want users to report them. I just need a php script ( i can integrate it myself) that renders a reCAPTCHA box and a send button. When the send button is clicked i receive an email with the URL from where the report click originated from.

Another idea is to also add a text box where users write the episode name and then press send. a different option in case the above one is kinda funky.
