I would like to know if it's possible to add images next to the item title

In my case I am trying to make a download portal with K2 and would like to show a small image next to item name (at the left) in a similar way like it's being done in download portals

You will have to add the code to display the icon image into the view you want in an override. use category_item.php to make something similar
Somewhere here there is a plugin to allow multiple images to an item. That may be helpfull, or you could set the xsmall size to be (very small) and just use the item image
