I have seen this done in a Joomla template, but I don't know how to accomplish this -

If I don't upload a specific image for item, the first image in article would be taken and used as an Item Image, including all category views and other possible instances....

Hello you can search for the IAKI (Import As K2 Image) plugin in the extend K2 section of this site.
