Can you advise what the problem was and what caused it?
Was it caused just by the installer?
How can we check and what to check in Version 2.5.4 that is on our clients site?

Some folders were created by the media manager with 777 permissions, no need to worry.

I do however suggest you always keep you components and Joomla! up to date.
