In the front end, when editing an item and the "Add Image" tab is selected, the box attaches to the top of the screen hiding the "Save" button.

Hi. Probably a CSS rule from your site is causing this. Unfortunately we cannot tell for sure by looking at the screenshot. Use a tool like Firebug to detect the CSS rule that is placing the modal on the top of your screen.
