I have chosen K2 - Categories as menu type, but then the heading for the site title is wrong font size. When showing source code for the site, I can see that the heading is set as div class componentheading. It works great on my other sites which are not using the category K2 menu type.

This is a template issue.
You need to paste the .componentheading class' attributes in to the div.itemHeader h2.itemTitle class (k2.css line 154)

Should I replace div.itemTitle with div.componentheading or should it be added in addition?

Is it only necessary to do that in line 154, or is there any more lines to edit?

You can either the element directly in your overrides or even better c/p one's CSS attributes into the other class.
