I'm using K2 items to insert music files which are read by a custom module that creates a music player. Is it possible to trigger a hit on the K2 item when the music is played?

When you mean a hit, you mean as in a event in your Google Analytics or Joomla!'s hits?

I mean a hit on Joomla!'s hits.

You would have to trigger to a custom script which increases the hits cell on the xx_k2_items table.
