The form is rendered but nothing appears.
Download a fresh copy of K2 and read this post: getk2.org/documentation/tutorials/174-templating-with-k2-and-the-concepts-of-sub-templates

Use it a guideline to locate your template's files (item.php and item_comments_form.php) and check them for any differences in the comments sections.
