When I set the form to enter comments to all users it shows up beautifully below my blog posts. However, when I set it to registered users only, it doesn't show up at all. Do you have a way that I can provide you with private login information so you can please take a look?

If you logged in and the form is not visible, then this is a template issue. You should address this issue to the template's developer.
