I have a question,

starting off today i notised that the new item screen is messed up.



Nomaly above the input there are some tabs with the text now left on the screen.

every thing is reacting wierd.

please can someone have any idea where to starting search to solve this problem.

Clear your browser's cache, if the issue persists then several extensions are known to cause this issue, K2 Multi Images being one of them.

do i have to disable the extensions or uninstal ?

Disable them one by one until you find the culprit.
If the issue is only related to the frontend, you can temporarily remove the itemform.php override (if it exists).

What is the consequesis to delete itemsfrom.php ?

Deleting itemsform.php solves the problem

None, if you delete the override then the default is being used instead.
Just make sure NOT to delete the default file :)
